At University Hospitals Elyria Medical Center, we understand that a diagnosis of heart failure can sometimes be frightening and confusing. That’s why we created the Heart Failure Clinic. Located in Suite 305 of Gates Medical Building (in the same suite as North Ohio Heart, Inc.), this unique facility provides treatment and education for those with heart failure. This close proximity enables more collaboration between our advanced-practice nurse and several highly-respected cardiologists.
The goal of the UH Elyria Medical Center Heart Failure Clinic is to “demystify” the term Heart Failure, and for patients to come away from their appointments feeling more in control of their disease. Education about medicines and symptom recognition is provided, along with assistance in identifying barriers to compliance, such as cost, dosing schedules and any perceived side effects.
Patients are referred to the Heart Failure Clinic by their physicians, typically family practice, internal medicine/primary care, as well as cardiologists, nephrologists (kidney) and pulmonologists (lungs).
The Heart Failure Clinic has partnered with UH Elyria Medical Center Home Health Care for congestive heart failure monitoring. Patients’ blood pressure and weight information is transmitted automatically to a home health care nurse who reviews it and then sends it to the Heart Failure Clinic for any recommended adjustments.
