Scheduling & Registration
The following information is provided to give you an overview of what to expect when you come to EMH. Please don't hesitate to call us at 440.329.7576 at any time if you have any questions or need additional information.
What Do I Need to Bring?
- You must have a complete physician's order:
- Name of test
- Diagnosis or reason for test
- Doctor's signature and date
- Current insurance card(s)
- Driver's license
- Credit card, cash or check for your deposit or insurance co-payment amounts.
Note: Identity theft in the health care setting may compromise patient care and could create life threatening situations. EMH takes patient safety very seriously and has an Identity Theft Program to protect our patients. For this reason, patients are expected to provide a photo identification when presenting for services. If you do not have a photo ID, we may require either a photo or fingerprint be taken.
Can I Pre-Register?
Yes, you can pre-register. You can call the Central Scheduling Department at 440.329.7576 during regular business hours (Monday - Friday between 8:00 a.m. and 5:00 p.m.). Bring your complete order and your insurance cards(s).
What do I Do If I Need to Cancel or Reschedule Appointments?
Call the Central Scheduling Department at 440.329.7576. Leave a message if it is after business hours. All messages will be returned within one business day.
How Do I Get There?
Where Do I Park?
Visitor parking is free. Parking includes designated spaces for people with disabilities. Access to EMH Elyria Medical Center from the parking garage is available by the pedestrian bridge to the ACC (Ambulatory Care Center) building.
